THE SCHOLARSHIP APPLICATION MAY BE COMPLETED ON LINE BUT A COPY OF SAME SHOULD BE PRINTED OUT AS THERE MAY BE SECTIONS THAT SHOULD BE COMPLETED BY YOUR GUIDANCE COUNSELOR OR ADVISOR. 

Student Assessment Form (Three required)

tHE aSSESSMENT FORM MAY BE COMPLETED ON LINE AND E-MAILED TO THE FOUNDATION. HOWEVER, IT IS SUGGESTED A COPY OF THE COMPLETED FORM BE PRINTED FOR YOUR RECORD IN CASE THERE ARE ANY TECHNICAL DIFFICULTIES.

(Graduating high school seniors may submit 3 of the Teacher evaluation forms included in the common application.)
